Can I tell you the sentence I hear most often in this room?

It comes out quietly. Almost always with a small apologetic laugh, like she has just admitted something careless.

She hasn't. The women who say it to me are the opposite of careless.

They researched the practice. They read every review. They asked two friends first.

Then they called, someone booked them, and on the day a person they had never met walked into the room and picked up a syringe. And by then it felt rude to ask who she was.

Because nothing has actually gone wrong. It was fine.

Fine is the word, and it is doing a lot of quiet work in that sentence.

Fine is the tox that had worn off on one side by week six, and you decided that was just how it goes. Fine is the unit count somebody quoted you over the phone before anyone had looked at your face. Fine is a result nobody complimented and nobody questioned.

And underneath it is the thing you actually think about, which is the woman at your club whose work you can read from across the room. In a town this size, if it looks obvious, everybody knows by the weekend.

It is not the product. Tox is tox. It is the same molecule in every clinic in Georgia and South Carolina, and nobody has a better version of it.

It is the dose, and where it goes, and whether anybody watched your face move before deciding either one.

Almost nobody's brows sit level. Almost nobody's smile pulls evenly. Put a standard number into a face that isn't standard and you get a result that announces itself.

Which means the variable is not luck. It is a decision. And decisions get made by a specific person.

Keri had a specific anatomical problem that two previous rounds somewhere else had not accounted for. It took one appointment of actually looking to find it.

Most people have never read this, and there is no reason they would have. But the Georgia Board of Nursing has a position statement on exactly who may perform cosmetic procedures, and on what has to happen before a nurse injects anybody.

Read that second line again, because it is the whole thing.

An individualized order. A history and physical. For you, not for everyone who walks in that week. Georgia says a blanket standing order is not good enough.

Which means a unit count quoted to you over the phone, before anybody has looked at your face, is not just a shortcut. It is the exact thing the state says does not count.

South Carolina writes it a different way. Its Boards of Dentistry, Medical Examiners and Nursing say that if a registered nurse is performing the injection, it has to be done with the on-site supervision of a physician or an APRN.

I am an APRN. That is what the DNP and the FNP-C after my name mean. So in this building, the person who does your history and physical and writes your order is the same person who is standing there when it happens.

You will know who is treating you and what she is licensed to do before you ever get in the car. It is me, or it is Alyssa Long, and you will be told which one when you book.

When you arrive, someone sits down with you before anything is drawn up. I will ask you to raise your eyebrows, frown, and smile, and I will watch which side of your face does what.

Then you hear two lists. What I would treat, and what I would leave completely alone.

That second list is the one that keeps you undetectable, and it is the one almost nobody bothers to make.

I start conservative on purpose, every time. You can always add a little more in two weeks. You cannot un-inject. That is the guardrail and I do not move it for anyone.

Your husband says you look like you finally slept. A friend asks whether you did something different with your hair.

That is the whole job. Not a new face. The one you already have, on a good week.
